Tepeyac: Making Preventive Healthcare Services More Accessible

Tepeyac Community Health Center continues to face the principal challenges that it began addressing 27 years ago: families experiencing barriers to healthcare and preventive health care services.

 

Throughout 2021, Tepeyac has undergone tremendous growth and evolution thanks to the support from generous funders and donors. Through this generosity, Tepeyac has been able to provide high-quality, affordable, and accessible primary and preventive care to Denver’s most vulnerable communities. Services ranging from primary care, behavioral health, dental, pediatrics, pregnancy, HIV services, nutrition programs and more.

 

As a recipient of many health-related awards, including the 2020 Million Hearts Hypertension Control Champion, Tepeyac aims to be a beacon of wellness for entire communities. We do so by helping patients with hypertension, and by providing coaching for chronic disease management and prevention.

 

During the COVID-19 pandemic, Tepeyac created opportunities to reach out and educate Latino and underserved communities in Denver. Such efforts included a door-to-door campaign knocking on 3,371 doors, building rapport and trust through face-to-face conversations with a total of 1,559 community members (more than half identifing as Latino or Hispanic), and recording 845 vaccinations at neighborhood sites.

 

By working in partnership with our patients and communities, our providers build relationships to support patient wellbeing and lifestyle. This kind of patient centered, community focused, and preventative care greatly reduces costly trips to the emergency room and local hospitals.

 

Tepeyac provides over 17,000 patient visits per year, resulting in $6 million in annual savings to the overall health system by treating our patients with care and respect.
