Notice of Intent to Destroy Medical Records

Tepeyac Community Health Center, in keeping with current state and federal regulations governing the retention and destruction of patient health records, has a retention period for medical records stored in paper format of 10 years after the most recent patient care usage of the medical record. Medical records of minors stored in paper format are kept for the period of minority plus 10 years (i.e. 28 years minus the age of the minor at the time of the most recent patient care usage of the medical record).

Notice is hereby given of our intent to destroy, by shredding, paper medical records generated prior to 2015 that have met these retention period requirements. These records were generated at La Clinica Tepeyac, Inc. facilities located at 3617 Kalamath St., Denver, 80211 and 5075 Lincoln St., Denver, CO 80216.

Destruction of medical records is scheduled to begin January 2, 2026. If you wish to pick up your medical records before destruction is set to begin, please contact us at 303-458-5302 and ask to speak to our medical records department or email us at records@tepeyachealth.org.
